When managing a patient with an acute psychotic episode associated with schizophrenia, our first priority must always be to rapidly stabilise them and provide effective control of their positive symptoms. When managing schizophrenia in the long-term, we need to ensure sustained control of symptoms and prevent psychotic relapses, while at the same time offering drug treatment that patients can tolerate and are willing to take probably for the rest of their lives. If we offer effective antipsychotic medications that provide reliable and sustained symptom control, we have a better chance of engaging our patients in psychosocial rehabilitation programmes, and by doing so, help them to avoid homelessness and unemployment, the breakdown of their relationships, substance misuse, and a possible decline into antisocial behaviour and criminality.
